It could be draped over the shoulders and neck, and there is an extra extension strap one could use to accommodate a big belly, etc. It can get too hot if you aren’t careful, so I don’t heat it to the max bc it could burn me. So then the heat doesn’t last as long as I would like it too, but overheating is dangerous for the person using it, as well as for the pad itself. The directions caution that the microwave must be completely clean (of course) bc any food residue that gets on the fabric will stain it and possibly cause it to catch fire. It is only spot-cleanable, not washable. The cover isn’t removable. I put it in the microwave folded in half, on a double paper towel for 2 and 1/2 minutes, then fold it the other way and do 2 minutes. It’s hot enough that I have to put it over my sweatshirt for a couple of minutes, then move it to over my t-shirt.
