How much of a gap can this cover? The floor is dropping. One side of the door is 1 1/2 inches and the other side is 1/2 inch

No, this will not work with a large gap as you described. To the best of my knowledge it works on approximately a 3/8” gap.

will this item surface mount?

Yes, it does surface mount at the bottom of the door. It is very easy too mount to the door.

Can the door sweep be mounted on the door exterior to prevent rain from coming in under the door?

If you put a piece of wood or Azek with an angle (45 for example) on top of the bar, the water would not accumulate and you would have a great product on your door.
