how do you mount this product? Glue or screws?

There are 2 mounting options. Permanent waterproof adhesive or 2 screws with wall bracket.

I need to buy the round adhesive pad. My original patch lasted 27 months. Help???

Mine came with the adhesive pad pre-attached to the bracket, making it impossible to use the screw mount options. Is that how everybody is getting it?
