Do you have to use the trackball or will the cursor respond to mouse movement?

If I understand you correctly. The mouse itself is stationary. This is a trackball mouse. The soul use of this is spinning the ball to move the cursor. I am unsure of any hybrid mice with the features you describe tho it made me wonder myself. You can always keep a regular one plugged in next to it.

Any right handers using it to relieve carpel tunnel and repetitive stress injuries in the right hand (by switching to the left)? If so, feedback??

Been using the Logitech m570 on right hand + standard Logitech wireless mouse on left hand for years. It's helped a lot with wrist pain. Finally decided to try this Elecom left handed trackball as it doesn't seem like Logitech has any intention of making a left handed m570. Might get the right handed Elecom eventually as well.

Will this mouse work with windows 10?

Yes it works great with Windows 10 still having a problem right scroll wheel 2 move across a row in Excel 2013 worksheet sheet. Very smooth Mouse
