How many cats are in the box?

Great question! Thinking in terms of Schrödinger's cat, you will find one cat in the box, said cat may either be alive or not. Mine was not alive when we opened the box, but quantum physics may be in your favor and you'll get a live one. :-) Best of luck to you!

Would anyone recommend the joy for all companion pet to someone who has depression and anxiety?

A million percent! I am a young adult (so not the target audience here) but struggle with anxiety which is why I got this kitty. And I LOVE it. Petting the soft fur and having the kitty purr helps my anxiety so much. Whenever I’m anxious I can just reach over and pet this cat and I feel so much better. Definitely recommend

Should you tell the elderly person you are giving it to that it’s a real cat?

I didn't tell her anything about it being real or not. It reacts when you pet it, purring, blinking it's eyes meowing, etc. She would talk to it a little, but mostly just enjoyed holding and stroking it. Sometimes she seemed to think it was alive, and it seemed to give her a lot of comfort (she was a life long cat owner). I think each person with dementia has moments of varying clarity. Just let the person you're giving the cat to enjoy it for what it is to them at any given moment. BTW... It has a switch to turn off just the sound if it gets annoying or is too loud, and you can also switch the cat off entirely.

how long does the batteries last?

I bought the creamy white cat September 2018 and still have the original batteries (12/2019). I must say the "cat" doesn't get played with every day so that may account for the longevity. I would say on average he is petted about twice a week for about an hour each. He also meows when you walk by or the lights change (brighter). You have the option to mute and turn the cat off if you want thus saving battery life.
