I am so sorry, I just saw your question. It comes with a seal and 2 bolts.

Does it come with all the parts in the back of the toilet

You get the gasket, flange bolts and toilet. Nothing to hook it to water.

Does it come with everything you need for installation?

the only thing I had to do was adjust the water line connection . I had another brand of comode and the bolts fit good. the thetford works good . good luck to you.
