Can it lay flat without the foot wells so one kid can lay down?

No, the foot wells are built into the bottom of the wagon. You possibly could fill the foot well with blankets or pillows so a child could lay down.

How does it do in rough terrain, sand, grass, also up hills over curbs, bumbs and holes?

We used this for the fair. It went over grass pretty well… the little electrical cords they have all over for the rides it went over pretty smooth however if there was more then one cord together, my husband had to pull the part of the wagon while I pushed to get over those ones. I’m guessing for tall hills you’ll need help from another person, but small hills it will go over fine.

Do the seats recline?

The back of the seats are the ends of the wagon, so no, they don't recline.

Can you put a carseat adapter on or in it?

The wagon came with seat belts.
