I'm giving this a 5 star rating and here's why. These things take some adjustment to get right. It took me three days of trial and error to finally get the correct pressure where I wanted it. First, the cushion supplied doesn't work for me. I'm pretty skinny and the cushion was too large and round. It put too much pressure on my hernia at first and I had to slice it in half in an attempt to reduce the pressure. The inside foam is a folded diamond shape which I cut into 2 triangular pieces. It felt better with only one triangle of foam in the pocket but again I had a little pain. My hernia is a righty and it is actually quite low and very far over to the right. This meant that the foam cushion was not directly over my hernia, it was slightly to the left of my hernia and wasn't putting pressure directly over it, instead it was kind of nudging it to the right which became quite painful. I finally overcame this by completely removing the cushion. This made all the difference. I managed to get more pressure over my hernia by tightening the right hand strap more than the left hand strap. This applied the pressure exactly where I wanted it. I wear it with the belt hitched up high behind me (on my lower back) which brings the front plate lower down, where I needed it. So, don't give up immediately, these things take some trial and error to get right - we are all different. For someone whose hernia is placed differently to mine, or someone who has more body fat, the cushion might well be useful. Again, don't be afraid to cut it out and chop it - once it is in the pocket and you're comfortable you never have to touch it again. You can even replace it with other material. Another factor is your choice of pants! Wear baggier rather than tighter pants otherwise your undercarriage gets a bit squashed! Another great feature - going to the toilet is easy. Unstrap the two velcro front straps, pull them through and behind you and tuck them in and you're ready to go, then pull them back through your legs and strap up once you're done. The elastic on the front straps "could" be a bit stronger. So far I have found them to be sufficiently strong but maybe they'll stretch over time, so far so good. Sizing. I'm a 33 inch waist but I bought the small sized belt despite my waist-size falling into the medium category. For me it works perfectly, the belt is nice and tight. Any more than say 34 inches and I would take the medium. This is my second inguinal hernia, I had one on the left about ten years ago and with the help of a belt and some specific stomach exercise I got better without surgery. I aim to do the same this time. Good luck people!

fits perfectly, with use I expect it will wearout but it's for short term purpose, happy with the product, helps me do daily things that I couldn't do without it
