how do I know what side I need

I have a 33" waist and I bought the medium size. It's perfect for me but for someone with a much larger waist it would be hard to use. I'd guess for a waist of maybe 40" or less the medium would be good and for bigger, the large would be good.

How many calories are burned using weighted hoola hoop

We apologize. We only know the hoop can trim up to 2 inches in 2 weeks if used 5 minutes per day. We do not have any data on calories burning, yet.

Does this come with a video or just a pamphlet?

No video, but the pamphlet is very knowledgable. It is also very easy to assemble and disassemble. I love it as I lost inches on my waste in a very short time.

How does this differ from the acu hoop® 4m - 4 lb. medium listed for $29.70 on the sports hoop offical store?

This is the same as Acu Hoop 4M. Amazon is just more expensive due to its fees.
