It means made in the USA. It doesn't always mean formulated and/or packed in the USA. Most companies now will actually state, Made, formulated and packaged in the US If they are truly a U.S. company.

Why is magnesium in the threonate form at such a much higher milligram dose than all others? like over 4x higher.

Magnesium assists in over 300 processes in the body, mostly neurological. There are various forms of magnesium depending upon how it is processed (i.e. magnesium oxide: magnesium lysine glycerinate chelate, etc.) I take this form at 2000 mg after reading research regarding its effect on brain functions. You will need to do your own research regarding why you are taking magnesium. Magnesium as a supplement is important.
