Mine also says 49/250, 3/10, whats up with that?

I don't know the answer, but I can say that my packaging has the same number designations--49/250 and 3/10--as you describe. It may be a manufacturer inventory code?

Is this a real hot wheels or is it custom?

The one I got seems legit, except it also says it's 49/250 and the official Hot Wheels site by Mattel has no mention of it. I don't care that it's not official since I only have it as decoration. I'd love to see what Mattel has to say about it, though.
