It’s single and not dual. Either way I was still going to use it.

what would this be used for?

It's used to attach a camera/lens or small telescope to a mount (usually on a tripod) and allow space to move the configuration to a balance point utilizing the slots in the plate.

What is the maximum distance between the two screws?

The 2 screw holes furthest apart are about 10-1/2"

Impossible to tell from pictures. Is this double sided (i.e clamps can be attached to both sides) or single sided ?

It is dimensionally symmetric, but only one side has stop screws on the ends. The other side has no stop screws, but does have rubber anti-skid pads which would need to be removed for a clamp to get a "full bite".
