yes it does but it does not match factory utv rocker switches

Q: What size hole is needed for round switch included in harness?

Not sure but it there’s a template and it says it on there

Q: Is the switch to turn on and off the lights waterproof?

No it's not waterproof. It does have a seal around it but it's not 100% waterproof. If it got a little water on it it should be fine but you should not submerge it.
